  2. Click the icon for Non Catalog & Procurement forms.

  3. Click the icon for HSC ITS Multi-Device Purchase.

  4. Depending on your browser, an external page connects, or the form is downloaded onto your computer. Open the form.

  5. For each device, complete the following fields. (You may enter up to 15 on this form.)

    1. Device Type

    2. Device Recipient's name

    3. Recipient's email

    4. Recipient's Contact Phone Number

    5. Room number where this device is to be installed

    6. Additional software that will need to be installed

  6. Save this form.

  7. Create a Non Catalog requisition. 

  8. Attach this form to the requisition.
