The Project Request Form is used to define needs and formally request a project. After submitting the request, an intake process will initiate to better understand the project scope, streamline project initiation, and improve communication between the customer and the project team. Below is an example of the project request process:

  1. The Project Request Form is submitted.

  2. The Project Management Office (PMO) reviews the request and gathers additional information.

  3. The request is approved by the sponsor and ITS leadership.

  4. Project prioritization is determined.

  5. The project is launched.

Here's an overview of the key fields and benefits of the Project Request Form:

  • Project Name: Please utilize this field to provide a name for the project so that it can be identified by the Project Management Office (PMO). If a name has already been selected or is known effectively across the campus, please enter that name here.

  • Problem or Opportunity Statement: A starting point to capture essential details about the project, such as its objectives, and timeline.

  • Business Impact: This field serves to collect comprehensive information about the impacts that the project will have on business unit performance.

  • Recommended Approach/Solution: Please articulate your vision and goals for the project.  Provides an opportunity to propose the preferred course of action or methodology to successfully execute the project.

  • Describe best practice for implementing the process or system: Provide a high-level outline of the implementation process or any complex tasks that are known at the current time.

  • Known Risks/Barriers: This field is designed to help communicate concerns, constraints, or specific risks your project may anticipate.
