003-Update the Disclosers Disclosure Profile

Update Disclosure Profile

1. Log in as your assigned discloser.

2. Open the Disclosure Profile for your assigned discloser. Note the current state, Action Required.

3. Read the content of the Instruction Center.

4. Click the Edit Disclosure Profile, then complete the SmartForm with the responses listed below. Complete all other fields as you like.

5. Instructions and Policies Page

I certify that I have read and understood…: Check the box

1. Entity Disclosure Information Page

   1. Do you have any financial interests…: Yes

   2. Entity disclosures: Click Add

      1. Entity: Abbott Laboratories

      2. Relation to discloser: Check Self

      3. Disclosure types: Equity

         1. Do you own stock … in this organization? Yes

         2. Enter the current value…: 50,000

         3. Do you own stock options…: No

   3. Complete Disclosure Profile: Read the notes on this page. Do not click Complete Disclosure Profile Update here because you’ll do that from the disclosure profile workspace. Click Finish.

2. Confirm you met the following success criteria:

Under the Disclosures tab, you see the Entity that you just entered.
