After completing the form, it will automatically be sent to the appropriate Central or Expert Business Office for approval. Re-activate expired responsibilities
When employees are setup set up with MAP access, sometimes their access is sometimes given an end date. Complete the Business Information Service Request to re-activate your responsibilities. You or your supervisor can complete this form. Select Add or remove responsibilities then choose the responsibilities needed for the account.
