Amend an IACUC Protocol
An amendment is any change to an existing WVU IACUC approved protocol (e.g., adding drugs, addition of animal numbers, adding personnel, change to procedure rooms, etc.). If you need to make changes to an approved protocol, you can begin an amendment from the protocol document view.
Amend a Protocol
From the Manage Protocols list in the WVU+kc IACUC module, click the Title of the protocol you wish to amend to open the document.
In the actions menu in the upper right corner of the page, click the Amend button.
A new Amendment section will be added at the top of the document. Read the instructions for completing an IACUC protocol amendment carefully.
At the bottom of the amendment instruction click in the text field, or click the Edit button to write a brief summary of the amendment.
If you clicked Edit to enter or change the summary, click Apply Changes to save and close the field.
You will now need to make changes to each section of the protocol document, as required by the details being amended. Click the General Information Jump to... link.
Verify that the Principal Investigator, Lead Unit, and Title fields are still correct (click Edit to change any field).
Click Edit in the Legacy Protocol Number field, if you want to add an old protocol number.
Type or paste-in the legacy protocol number, then click Apply Changes. The General Information section should appear as complete (check-marked) when all fields are filled.
When the protocol document has been appropriately updated, click the Submit button in the actions menu on the right. Correct any verification errors that appear.
Only one amendment or renewal can be submitted at a time on the same protocol. If the protocol contains no verification errors, the Status will be updated to show Submitted for Review.
